At Yazdani Family Dentistry, we love seeing new smiles. If
you and your family have decided to join us as new patients, then welcome! We
are so happy to have you with us and promise to treat you like family!
Upon
completing the new patient exam, you will receive a complimentary annual
whitening take home kit or an annual sports guard! Regular fees will be charged and some restrictions apply.
In order
to get things started as quickly and easily as possible, there are a few things
we would kindly ask you to do.
Patient Registration And Release
We want to
make sure that we have all the information about you and your family that we
need. That way we will know all the personal considerations that you and your
family needs. This will help us make sure that the care we give is as pleasant
and comfortable as possible when you come for your first appointment.
If you and
your family are switching to us from another clinic, it would be best to
authorize the release of your medical records. By giving us access to your
medical histories, our dental team will be able to pick up where you left off
with your old dentist and do their best job catering to all of your individual

New Patient Exam
If for any
reason you are unable to provide us with medical records for you or any member
of your family, you will need to book an appointment for a new patient exam so
we can complete your patient profiles.
We will conduct a comprehensive examination of your mouth and teeth to find any
problems that we will need to fix, a thorough cleaning, and take x-rays. Please

needs one.
What to Expect At your first appointment
When you
first come to the clinic, expect to be greeted by our helpful and friendly
receptionists. They will get you signed in. If you didn’t already fill out the
patient registration form, they will have you fill it out at this time.
When we
are ready for you, it will be time to meet your hygienist, dental assistant and
dentist. They will have a quick chat with you to get to know you better and
help you settle in and get comfortable before they get to work. If you provided
a medical history, your new dentist will review it with you at this time. We
will then provide an examination, clean and polish your teeth and take x-rays
if needed. If the dentist finds any problems that need to be fixed, they will
let you know what needs to be done and have the receptionists help you with
booking a follow-up appointment.
At this time, they will also get any billing
information from you such as dental benefits or insurance details and take care
of the payments that need to be made. From there, you’re out the door, getting
on with your day, and on the way to your best and brightest smile!
